Certified copies are often required for situations where presenting the original is not possible.

What is an Acknowledgement?

 An acknowledgement of execution is used for legal agreements, deeds, powers of attorney, bills of sale, business documents, corporate acknowledgements, etc. 

Notarization of form DS-3053, and certification of valid photo I.D. for submission with a minor's U.S. Passport application.

One document, AZ notarial fee, identity verification of signer(s) and witness(s), travel (within 10 miles of home base if requesting mobile services)

Multiple documents, AZ notarial fee, identity verification of signer(s) and witness(s), travel (within 10 miles of home base if requesting mobile services)

For services outside of our normal business hours or on weekends/holidays there will be an additional flat charge on top of your document package.

Document Notarization or Acknowledgement for Veterans and Active Duty Military, includes document notarization and verification of identity. Valid ID showing Veteran or Military Status Required. As the spouse of a Veteran I have access to Davis Monthan so can come to you on base.
